Supporting the Home Health Operations team, the full-time Home Health Referral Care Coordinator will coordinate referrals, communicate with providers and injured workers, and ensure timely access to home health and IV therapy services in a fully remote environment. Key responsibilities: Manage home health and IV therapy referrals, assisting with intake and documentation Promote positive relations with referral sources and ensure accurate demographic data and documentation Coordinate with local vendors and negotiate rates for necessary services to support care coordination Required qualifications: Some college education and/or medical background; LPN preferred Experience with Work Comp Insurance, referrals, and authorizations preferred Proficiency in medical terminology and moderate PC skills, including MS Office Ability to work collaboratively with co-workers and external customers Knowledge of injured workers' rights and commitment to confidentiality and privacy
