Provide information to outpatient substance programs.
Provide patients with adequate resources such as housing, food, and legal services. Based on the SDOH.
Referral to community agencies. Meals on wheels.
Referral to Health Home.
Call patients that were Discharged from the ED.Asst inpatient Social Work staff without patient resources.
EMR knowledge and skills – EPIC.
Documents patient care navigator notes.
Email/telephone knowledge and etiquette.
Respond to incoming emails and calls in a timely manner.
Ensure out of office notifications are activated on scheduled days off.
Review emails and voicemails on a daily basis.
Education Required:
Associate degree at minimum.
