Join the inpatient Care Management team at St. Cloud Hospital in the role of Float Social Worker. This position allows the ability to float to all units of the St. Cloud Hospital!
Our Socials Workers at the St. Cloud Hospital serve patients and families in our community and beyond by helping them during a challenging time in their lives. Our Social Workers enjoy the variety of cases they are presented with as there is always something new and different. You'll collaborate with other Social Workers to gain knowledge and experience and work with an interdisciplinary team of nurses, therapists, physicians, and dieticians. You will have a designated patient caseload and will plan care to effectively meet the patient/family transitional needs including monitoring the length of stay and promoting community resources.
We will provide supervision for new grads in-house or will reimburse up to $175/month for external supervision.
Schedule
Full-time | 80 hours every two weeks | Onsite
Day shift | Monday through Friday | 7:30am-4:00pm

Pay based on experience and degree
Bachelor’s in Social Work starting at $32.09 per hour
Master’s in Social Work starting at $33.15 per hour
Pay range: $30.65 - $49.73 per hour
$3,000 hiring bonus and $5,000 relocation bonus for eligible candidates!

Qualifications
Current Licensed Social Worker (LSW) in Minnesota required.
Master's degree in Social Work or Sociology required.
At least two years of experience as a Social Worker, preferably in a healthcare setting.
American Case Management Association (ACMA) Case Manager-SW Certification required within five years.
