Position Summary: The Substance Abuse Counselor/Counselor Intern works as part of a multidisciplinary team to coordinate treatment services necessary to support recovery from Opioid Use Disorder. Responsibilities: Provide individual and group counseling. Completes assessment of each patient using approved assessment tools. Develop a comprehensive treatment plan. Documentation of patient progress in electronic health record. Facilitates the transition/discharge process, including arrangements for follow-up services.
